We have worked with colleagues at the University of Sheffield and Sheffield Hallam University to create a series of worksheets from our Your teeth, You are in Control dental anxiety book for teenagers. Central to that is our Message for your dentist worksheet, that allows you to pass on your requests and wishes to your dentist.
Please download for use in your own life, or for non-commercial work in your own dentistry practice.
